79-year-old man. Referred for chest pain. Previous cardiac failure. Severe coronary artery disease. Occlusion/stenosis RCA (venous graft in 1984). Severe stenosis LAD: LIMA-LAD (1984). Stenosis of LCx treated by saphenous venous graft (1984).

Opacification of the saphenous graft: sacculation of the vascular structure.

Final diagnosis


Pseudoaneurysm (double arrows) of the saphenous venous graft. Chest pain was probably explained by compression of the LIMA and induced ischemia.
