Clinical history

80 y-o Man. Tobacco consumption: 45 packet-years. Dyspnea and non productive cough for 1 month.

Radiological findings

Frontal chest radiograph. Retrocardiac opacity (star). Ill-defined left hilum. Relative hyperlucency of the left hemithorax.
Posterior part of the left hemidiaphragm obscured. Relative opacity of the lower dorsal spine (star).

CT findings

Axial CT sections. Left hilar mass obstructing the origin of the left lower bronchus. Enlarged lymph node (ATS11L) between the bronchus and the left lower lobe pulmonary artery. Left lower lobe (star) displaced medially and posteriorly against the costovertebral gutter. Small pleural effusion.
Consolidation of the left lower lobe (star) limited by the displaced medially major fissure.
Consolidation of the left lower lobe obscursing the left hemidiaphragm.

Final diagnosis

Fiberoptic bronchoscopy demonstrated a complete obstruction of the left lower lobe bronchus by an adenocarcinoma. Staging after PET-CT: T2N1M0. Surgery. Pathologic staging pT2N1M0.
