53 y-o man. Hemoptysis.

Radiological findings


Frontal chest radiograph. Left hemithorax opacity obscurcing the left heart contour. Note the visibility of the aortic knob (arrow).
Lateral view. Large anterior opacity limited posteriorly by a well-defined interface corresponding to the main left fissure.

CT findings


CT axial sections. The left upper lobe is almost entirely opacified. Stenosis of the left upper bronchus. The contours of the central mass are difficult to separate from the atelectasis.
Coronal CT reformation. The aortic knob is silhouetted by the unopacified lung of the upper segment of the left lower lobe (Luftsichel sign).
Sagittal CT reformation. The left upper lobe is displaced forward and is limited by the main left fissure.

Final diagnosis

Atelectasis of the left upper lobe. Proximal obstructive bronchogenic carcinoma. Staging T2N0M0 after PET-CT.
