Case 1

31 y-o woman. Cystic fibrosis. No current infection. Stable respiratory status.

Radiological findings


Obliteration of the right cardiac border. Possible downward position of the right hilum (arrow). Diffuse bronchial thickening. Triangular opacity superimposed on the cardiac silhouette (arrows), its top projecting over hili and its base located at the corner between the anterior chest wall and the right hemidiaphragm.

CT findings


Axial and coronal CT. Triangular opacity abutting the right cardiac border limited by the right major and minor fissures: middle lobe atelectasis. Diffuse dilated bronchi and centrilobular micronodules with tree-in-bud pattern.
Coronal and sagittal thick CT reformations. Dilated bronchi in the atelectatic right middle lobe (arrow). Note the downward position of the right hilum.

Final diagnosis

Atelectasis (obstructive) related to chronic bronchiectasis of the middle lobe. Diffuse bronchiectasis, bronchiolar micronodules and tree-in-bud related to the chronic airway disease.
